Texas Consumer Notice: Access to Health Records + Complaints
1) Request your health care records from this practice:
To request a copy of your records, please submit a written request using the contact information below:

Your full name
Date of birth
Phone number and email
What you are requesting (e.g., “entire record,” “treatment summary,” specific dates)
Where you want records sent (you, another provider, etc.) and any deadlines
If you want records sent to another person or provider, you may be asked to complete an authorization/release form.
2) Contact the Council (licensing / disciplinary authority):
